「DEAR BOYS ACT4 23」は、神奈川県ナンバーワンを目指す名門・湘南大相模高校バスケ部の活躍が描かれた作品です。インターハイ予選で宿敵・瑞穂高校に敗れた後、キャプテンを引き継いだ布施歩が新入部員とともに全国へと挑みます。PGの柳本が怪我で離脱し、青山と柏木の2年生が新たなPGとして活躍。さらに森もメンバーに加わり、チームは再編されます。インターハイ神奈川県予選では、瑞穂との再戦や横浜学園との激戦を乗り越え、2位で沖縄大会へ進出。沖縄大会では嘉手納西高校に勝利し、明和大日立高校との戦いに突入。超守備陣形の相手に苦戦する中、湘南大相模が打開するための“新たな一手”が注目されます。

『DEAR BOYS ACT4 23』follows the journey of the prestigious Shonan Dai Shimo High School basketball team, aiming to be the number one in Kanagawa Prefecture. After losing to their rival, Mizuho High School, in the prefectural preliminaries, the team, led by new captain Furihara, faces challenges as star PG Yanagimoto is injured. New members, including青山 and 柏木, step up to fill the gap. In the Kanagawa Prefectural Tournament, they advance to the final four, facing Mizuho again and eventually reaching the national tournament in Okinawa. In the Okinawa tournament, they defeat local team Kadena Nishi High School and face a tough battle against Meiwakadai Hitachi High School, known for their strong defense. The team must find a new strategy to overcome this challenge and move forward.
